Intake Coordinator - Home Health

Great Home Health company is hiring a Scheduler/Intake Coordinator. You must have Home Health or Hospice Industry experience to qualify.
You will manage all aspects of the patient intake process, including verifying benefits, collecting patient information, and scheduling home health visits. They also coordinate staff schedules to ensure timely and effective care.

Key Responsibilities:
  • Intake Coordination:
  • Obtain and verify patient and insurance information.
  • Log referrals and maintain accurate patient records.
  • Enter patient information into electronic health records (EMR).
  • Scheduling:
  • Develop and maintain schedules for home health visits, considering patient needs, staff availability, and care plans.
  • Coordinate schedules to ensure consistency and minimize overtime.
  • Communicate with patients, families, and caregivers about schedule changes.
  • Communication and Coordination:
  • Act as a point of contact for clients and caregivers.
  • Address client concerns and provide information about home health services.
  • Collaborate with other healthcare providers to ensure coordinated care.
  • Administrative Tasks:
  • Maintain accurate records and reports.
  • Perform general office duties, such as filing and answering phones.
Required Skills and Qualifications:
  • Experience: Previous experience in home health or healthcare, particularly in scheduling or intake, is highly desirable.
  • Communication Skills: Excellent verbal and written communication skills are crucial for interacting with patients, families, and staff.
  • Organization and Detail-Oriented: The ability to manage multiple tasks, prioritize effectively, and maintain accurate records is essential.
  • Computer Skills: Proficiency in using electronic health records (EMR) and other computer applications is necessary.
  • Interpersonal Skills: A friendly, helpful, and patient demeanor is important for interacting with clients and caregivers.
Additional Notes:
  • Some positions may require on-call hours.
  • Knowledge of relevant regulations and standards, such as those of the Centers for Medicare & Medicaid Services (CMS), is also beneficial.
