Key Responsibilities:
Supervision and Staff Management:
Hiring, training, supervising, and evaluating rehabilitation staff (e.g., therapists, aides). Including assistance with managing clinical staff. This includes conducting performance reviews, providing coaching and mentoring, and addressing any performance issues.
Patient Care Planning and Coordination:
Developing, implementing, and updating individualized plans of care based on patient needs and therapeutic goals. This also includes coordinating with physicians, nurses, and other healthcare professionals to ensure a seamless and effective continuum of care.
Regulatory Compliance:
Ensuring that all rehabilitation services and documentation comply with relevant federal, state, and local regulations, as well as agency policies and procedures.
Quality Improvement:
Monitoring patient outcomes, identifying areas for improvement, and implementing strategies to enhance the quality and effectiveness of rehabilitation services.
Budget and Financial Management:
Developing and managing the rehabilitation budget, ensuring cost-effective resource allocation, and tracking financial performance.
Documentation and Reporting:
Ensuring accurate and timely documentation of patient care, progress, and outcomes.
Patient and Family Education:
Providing education and support to patients and their families regarding their rehabilitation plan and home care management.
Interdisciplinary Collaboration:
Working effectively with other departments and staff to ensure a team approach to patient care.
Quality Assurance:
Participating in quality assurance activities, including audits and performance improvement initiatives.
Essential Skills and Qualifications: Supervision and Staff Management: A licensed healthcare professional with expertise in rehabilitation, such as a physical therapist (PT), occupational therapist (OT), or speech therapist (SLP).
Management and Leadership Skills: Strong leadership, management, and interpersonal skills, including the ability to motivate and supervise staff, communicate effectively, and collaborate with others.
Clinical Expertise: Extensive knowledge of rehabilitation principles, techniques, and best practices.
Regulatory Knowledge: Familiarity with relevant regulations and standards for home health and rehabilitation services.
Communication and Documentation Skills: Excellent communication (written and verbal), interpersonal, and documentation skills.
Problem-Solving and Decision-Making Skills: Ability to analyze situations, identify problems, and develop solutions.
Computer Proficiency: Proficiency in using computer systems for documentation, scheduling, and communication. EMR Systems like WellSky, Kinnser, Oasis and MS Office Suite
