TheDirector of Client Care plays a pivotal role in overseeing and managing theclient/patientcare and ensuring that their needs are met with compassion and efficiency. This individual serves as a primary point of contact for both clients and caregivers, actively resolving any issues that arise and facilitating effective communication between all parties involved. The Director of Client Care has oversight over all other Client Care Managers and ensures the Department is running at optimal efficiency. The Director of Client Care is trained, qualified, and expected to ensure the Home Care Compassionate Memory Care Dementia class and other training opportunities that are being offered monthly. This role is essential in managing our client/patient satisfaction scores and maintaining efficient business operations.
This is Full time M-F schedule form 8:30 am -5 pm and you will be responsible for covering client care and caregiving matters 2 nights a week.
Responsibilities
Conducts initial client screenings to develop a care plan.
Creates a plan of care for clients that includes input from client and client’s representative.
Resolves client and caregiver complaints and other issues.
Travels to client residences to conduct investigations of issues as needed.
Responsibleformonitoringclients off schedule for hospitalization or other reasons and coordinating with the scheduling department.
Responds to changes in client condition reported by caregivers and nurses.
Participate in the quality assurance reviews and evaluation of agency services
Assist with answering incoming phone calls and talking to potential employees and clients.
As needed, assist with interviewing caregiver applicants and conducting orientation for them.
As needed, assist with scheduling client visits.
Collaborate with healthcare providers and community resources for comprehensive client care.
When on call, take calls after hours for emergencies, scheduling issues and client referrals/intakes.
Learns our computer systems for personalized care plans, communication with all parties involved, and other duties if needed.
Acts as a liaison to provide connection with the social service delivery services locally.
Provides light case management service to follow up on service delivery.
Provides quarterly assessments and documents client process and any updated needs on the needs assessment.
Detail case notes or documentation from each client contact.
Education & Experience Required
Bachelor’s degree (B. A.) or equivalent from two-year college or technical school preferred.
6+ years related experience or training.
Caregiving, nursing, or home care experience a plus.
Candidate must have intermediate computer skills and be comfortable with Wellsky.

Salary & Benefits
$95,000- $115,000 base salary plus bonus opportunity.
