The Director of Patient Care Services (DPCS) provides oversight and support of all patient care and associated personnel for assigned program(s). This oversight includes patient and personnel assignments, coordination of care, coordination of referrals, assessment of patient needs, and ensuring the plan of care is developed, implemented and updated. The DPCS is responsible for the services provided by the home health and hospice programs and strives to provide the highest quality of care through all service lines. The DPCS is responsible for staff engagement, development and performance, including policy, regulatory and compliance knowledge and adherence. The DPCS is always available during operating hours (or designates a similarly qualified alternate) and participates in all activities relevant to assigned programs including setting/developing and executing strategic goals, budgets and staffing models.
Responsibilities The DPCS is responsible for their assigned program(s) lines of service and overall results compared to goals. They will effectively ensure the facilitation of required Interdisciplinary Group Meetings, Case Conferencing, and Coordination of Care. They will investigate and report any problem relating to patient care or conditions which might harm the patient and/or employee well-being following appropriate processes.
Responsible for recruiting, hiring, evaluating and terminating personnel in collaboration with Human Resources. Directs the planning and completion of orientation for new employees which may include conducting selected orientation classes.
Participates in the Program’s ongoing Quality Assessment and Performance Improvement (QAPI) program through collaboration with the Compliance Officer.
The DPCS is a key member of the leadership team and works closely with other leaders to coordinate internal services with the services of other community agencies, to increase our presence in our service area and to ensure a positive and healthy working environment. Working in collaboration with the Business Development team the DPCS will identify opportunities to develop new lines of service and referral sources.
The DPCS will primarily be focused on internal marketing with respect to the excellence of the clinical program, great customer service, and expansion of clinical programs
The DPCS outreach goals shall include continued active relationships with providers and community activities that provide opportunities to grow census through identification of provider’s/patient’s goals and needs and ensuring our services are clearly understood.
The Director of Patient Care Services ensures compliance with state, federal, and accreditation regulations and guidelines in accordance with company policy. Plans for and ensure compliance with policies and procedures which meet Medicare and Medicaid guidelines
Ensures assigned staff maintains this same standard of compliance and performance and instils these requirements in their staff. Maintains and keeps all personnel file information as required by position.
Education/Experience/Skills/Qualifications Possession of a current RN license to practice in the state of California and current CPR certification. Three year’s experience as a Home Health Supervisor required, 5 years preferred. Ability to exercise independent judgment and critical thinking to supervise and coordinate clinical professionals and agency support staff. Ability to learn to effectively operate computer hardware and software programs.
