MDS Coordinator, RN

We are seeking an experienced Registered Nurse (RN) to join our skilled nursing team as an MDS Coordinator. This is an excellent opportunity for a detail-oriented clinician who is passionate about resident care, accurate reimbursement, regulatory compliance, and interdisciplinary collaboration.

Responsibilities

  • Coordinate and complete the MDS assessment process in accordance with CMS and state regulations.
  • Ensure timely and accurate completion of MDS assessments and care area assessments (CAAs).
  • Collaborate with the interdisciplinary team to develop and update individualized care plans.
  • Maximize Medicare, Medicaid, and managed care reimbursement through accurate documentation and coding.
  • Review clinical documentation for accuracy and completeness.
  • Monitor Quality Measures (QMs) and assist with quality improvement initiatives.
  • Maintain compliance with federal, state, and facility regulations.
  • Participate in survey preparation and respond to audit requests.
  • Educate nursing staff on documentation requirements and MDS processes.

Qualifications

  • Current Registered Nurse (RN) license in Connecticut (required).
  • Minimum 2 years of MDS Coordinator experience in a skilled nursing or long-term care setting.
  • Strong working knowledge of RAI Manual, MDS 3.0, PDPM, Medicare, Medicaid, and CMS regulations.
  • Experience with PointClickCare or other electronic health record systems preferred.
  • RAC-CT certification is a plus.
  • Excellent organizational, analytical, and communication skills.
  • Ability to manage multiple deadlines while maintaining accuracy.
