Apply to MDS Coordinator
We are seeking an experienced MDS Coordinator to join our skilled nursing team in Chicago, IL. This is an excellent opportunity for a detail-oriented clinical professional who is passionate about resident care, reimbursement accuracy, regulatory compliance, and interdisciplinary collaboration.
The ideal candidate will have a strong understanding of the MDS/RAI process, Medicare and Medicaid reimbursement, PDPM, and state and federal regulations.
Responsibilities
- Coordinate and complete the MDS assessment process in accordance with CMS guidelines and regulatory requirements.
- Ensure timely and accurate completion of MDS assessments, Care Area Assessments (CAAs), and comprehensive care plans.
- Collaborate with the interdisciplinary team, including nursing, therapy, dietary, social services, and physicians, to develop and maintain individualized resident care plans.
- Monitor MDS schedules and ensure compliance with all federal and state assessment timelines.
- Review clinical documentation to support accurate PDPM reimbursement and regulatory compliance.
- Identify opportunities to improve quality measures, clinical outcomes, and documentation practices.
- Educate and support clinical staff regarding MDS documentation requirements and regulatory standards.
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
- Stay current with CMS regulations, reimbursement updates, and industry best practices.
Qualifications
- Current Illinois nursing license (RN or LPN) in good standing.
- Previous MDS Coordinator experience in a skilled nursing or long-term care setting is required.
- Strong knowledge of the RAI Manual, PDPM, Medicare, Medicaid, and CMS regulations.
- Experience with electronic medical records (EMR) and MDS software.
- Excellent organizational, analytical, communication, and time management skills.
- RAC-CT certification is preferred but not required.
